1. What the decision was.
2. How long it waited before a named owner closed it.
3. Whether that owner had a stated trigger or decided under deadline pressure.
4. Whether it raised or lowered next quarter’s Integration Debt.

Cap the scorecard at eight rows a month. Nobody reads a longer one.
