Fill In the Monday Morning Page in Fifteen Minutes

This is the one page behind the weekly thirty-minute huddle in “The Operation Matters More Than Any Single Program.” Someone should be able to fill it in within fifteen minutes on Monday morning, before the first patient arrives. It has six sections, in this order, and each one comes with the questions it answers:

1. Capture Ladder by program. Which program’s ladder moved since last week, and which rung moved it?
2. Admin minutes per patient-month. Which way has the number gone over the last eight weeks? What did the team do last week that nobody recorded?
3. Open escalations older than their deadline. Which escalations are still open, who owns each one, and how old is the oldest?
4. Denials. What came back, and what did the denials have in common?
5. Patients who fell off. Which enrolled patients have had no contact in thirty days? Who went to the hospital or the emergency department last week, and has each of them been contacted? Which devices went quiet? Who asked to stop?
6. One thing to fix. What did we change last week, and did it work? What’s the one thing we fix this week, and who owns it?

If this page takes half a day to fill in, the problem is the information system, not the people. Fix the system first.
