The seven prototype tools and the diagnostic

Each page is a blank, reusable copy of one tool from the book, in the order the chapters introduce them. Fill in one copy per student, population, seam, meeting, or family. Each starts with a line saying when to reach for it and names the chapter section that explains it. The book prints every tool in full, in “Make the Seams Disappear for the Child” and “Test the Model on One Seam Before Reorganizing Anything.”

  1. Score the Diagnostic by Cluster Before Choosing a Seam: the Child Experience Fragmentation Diagnostic, thirty items in six clusters.

The diagnostic comes first because it points to the seam. The seven tools then follow the order of the book’s Figure 10.1: map the team, audit one seam, run one meeting, check with the family, hand off one transition, decide whether a dependency needs a routine, and audit whether a tier and core instruction form one system.
